Another Olympus Scope, a Bronchoscope, Associated With HIA
Another Olympus Scope, a Bronchoscope, Associated With HIApneumonia. This infection or pneumonia in 2 patients in an ICU with an unusual strain of Pseudomonas was recently linked to an Olympus bronchoscope that had been properly reprocessed with high level disinfection, did not have visual defects, but did grow the unusual Pseudomonas bacteria, (whereas 8 other bronchoscopes in the ICU did not grow the bacteria). The unidentified model scope made by Olympus was removed from service.

This is more than a pattern: another report of a small to large group of patients being infected by (unusual, or even superbug status) bacteria from a scope, (bronchoscope, duodenoscope, gastroscope...), despite the offending scope having been properly handled and disinfected.
The pressure is building on the FDA and industry to solve this aspect of the problem of HAI's, (Hospital Acquired Infections) from contaminated scopes, and TSO3's techology of terminal sterilization of scopes via the VP4 is a, or THE, definitive solution.
