This AR question was common to all Conference Calls for 18 months,.  The common answer was 100% collectable and that was always the case historically,. When AR became plus 52 weeks there common answer became "we and our Auditors deem it as collectable" and unfortunately no one questioned it at least publically beyond that,,. In hindsight its seems negligent to not have provided further color on this problem as it developed beyond 52 weeks,,.When you think about it, why didn't the Analysts press them on it during the calls,,.

I believe an earlier comment has merrit, that some insurance company has them over a barrel on a aspect of there service and the outcome is being negotiated, this was likely sensitive and kept out of the public, unfortunately this is standard operating proceedure in the US Healthcare Insurers,,.
