It seems reasonable to conclude that the government’s system of inspection and oversight must be seriously flawed, since it either failed to identify or to remedy the structural or operational defects which precipitated the dam’s failure.
So, we need an independent inquiry. The government says it means to have one, but we need a genuine arm’s-length inquiry, not some backside-covering exercise involving industry and government “experts” on whose watch this occurred.
We don’t need the fox and the watchdog interviewing each other to find out what happened to the chickens. We know what happened to the chickens. What we don’t know is why the watchdog failed to prevent it."
