EGRIFTA SV (tesamorelin for injection) is approved in the United States for the reduction of excess abdominal fat in people with HIV who have lipodystrophy.

You also have to appreciate the fact that there has been an enormous backlash against inflated drug pricing with drugs that don't have much competition for parts of the population here in the US that could really use them. So, it kinda makes sense on the pricing side to stay conservative...
