RE: For the canadian boys/girlsWolly, I was in Oilsands Quest when they were private, before the reverse takeover with Canwest.  When the reverse takeover was done, these shares were marked restricted.  My broker got me to fill out a similar form to make sure I could sell them whenever the time comes (hasn't come yet, go BQI!) - if you were also in when they were private, that's probably what's going on.  Your broker should be able to help you fill it out; the form confused me but my broker walked me through it and it wasn't that bad after all.

In the meantime, I've been trading in my discount account (TD) with no problems at all - no forms needed to buy new shares (market shares aren't restricted) or sell those ones - your broker shouldn't be freezing your account from buying market shares, or selling non-restricted market shares - if it is, maybe it's time to get another brokerage.
