It's on the right half of the proxy form, middle of the page.  The arrow from the "Control No: " points to it.

 

It's also at the left hand side, next to the CUSIP number.

You could call your broker or Valiant Trust if you haven't received your control number. Perhaps they could find it; I don't know.
