Cytta sees the patient-centered medical home ecosystem (PCMHE) as anintegrated network of eHealth technologies and services capable of beingutilized to organize primary care to ensure patients receivecoordinated care from a primary care physician supported by advancedmonitoring technologies for self-care management, provided by a skilledhealthcare team.
The goal of the patient-centered medical home ecosystem is to create themechanism to deliver continuous, accessible, high-quality,patient-oriented primary care to lower costs and improve quality of carethrough the adoption of health IT, care teams and evidence-basedmedical guidelines.
