I called Kingsdale last night and asked about exactly that.  After checking with someone else, the answer was that box is only for if you are voting at the meeting in person.  However, if you put your name there and vote online, the online vote counts and you won't be able to vote on the resolutions again at the meeting.  So you can leave it blank or put your name and it won't make a difference as I understand it. I agree it was unclear as well.

Everyone should call anyway.  I got a laugh out of it.  They have a FIU hotline setup to manipulate shareholders, and a completely outdated count of share holdings. At least that was the case for me.  Although it is possible they just checked one of my accounts and I misheard the person. 

So call just for fun, and if they are talking to you they won't be calling some poor uninformed shareholder they can manipulate.  When they asked me how I was voting I told them I didn't care to indicate at this time.
