I haven't received mine but here is the reply I received when I emailed the company hired to assist people with the voting process. I contacted them because I haven't received anything in the mail yet either.

Because voting instruction forms are only generated once and cannot be duplicated, the best way for you to vote your shares from outside North America would be to vote them online at www.proxyvote.com using your 12-digit control number. Your control number is located on the voting instruction form that was mailed to you with your meeting materials, but you can also request it from your broker or the bank you trade with.
