Yes, it has been happening with me as well. I believe it is a technical glitchon the SH side of the coin.
You click to ignore a poster ... there is a delay ... you then click on their name again thinking the first click didn't take. What happens I believe is that you actually do set that initial poster to your ignore list. However, when you click the second time, thinking that the original click didn't work, you actually end up ignoring the next poster that was on the forum under the initial target.
Thus, you are actually addint two posters to your ignore list ... the first one on purpose, the second by mistake.
