I would go one step further, why does the European division of BT work there better than here. They have had a few issues but nothing compared to the disasters here (take away Britain fiasco). It can't be just because the head office is there. Most of the trains and street cars I've used there work fabulous and the run them by the minute. Signs at stations tell you exactly when the train arrives, and it does. Not to mention they are wheelchair accessible.
Something in the manufacturing and leadership in North America is broken and nobody seems to want to address the problem.
