Is the $75k net of the fee healthcare institution pay upfront and or on an ongoing basis (for the software and service)? Is there any other highset up costs ?

Arizona to answer a part of your question you may have missed it, VH as an indirect partnership with Apple those facilities need to buy the hardware.
Just an example my wife is a RPN at Bruyere Hospital in the palliative care unit they have 32 beds, they operate with 2 RNs, 5 RPNs & 2 aids on her day shift so for Trinity Village as 150 beds I estimate they would need at least 25 to 30 IPhones that can be passed along at the end of each shitf let's say the #10 model at $1529 pretty sure they can get a rebate if they buy a big volume of smartphones.
$1529.00 CND x 30 phones = $45,870
75K / 12 months = $6,250 per month savings so your ROI on the hardware would be about 7 & a half months
