Evidence of efficacy will be almost immediately available to the researchers when the transplanted cells have engrafted and start producing insulin by themselves or are not producing insulin.
The study will have to monitor the subjects very carefully since the subjects continuous glucose monitoring equipment will be constantly telling the doctors what the patient's glucose level is.
I can see the doctors watching the requirement of continuing quantity injection of required insulin becoming less and less if the cells are producing insulin on their own.
I can also see it as being a series of medical judgement calls; since if the efficacy is sufficient the injections of insulin should become less and less necessary.
In fact if the cells in the pouch produce sufficient qualtity of insulin then the doctors monitoring will have to decide when they can and should stop injection of insulin.
that will be because the cells produce sufficient insulin to automatically regulate by themselves the quantity of insulin and glucose in the system and produce more insulin when needed and produce less when the glucose level starts to reduce below what the best level is. This will mean that the injuction of insulin beyond that then needed will cause the cells to throttle back to avoid glucose levels dropping below what the insulin producing cells consider to be desireable
in the best of all possible world the cells will automatically produce more insulin as meals increase glucose levels, and produce less as the glucose level falls below the best level as seen by the cells themselves.
Of course the question will be when do the doctors report the trial results to the company and when will the company report them to us as the shareholders. That is the big question
