It appears management was asleep.  The financials came out, the CFO probably swamped with calls to accounts payable and saw how hopeless the financials are and quit.

Everyone scrambled to contain the disaster to avoid receivership but it is too late and increasing interest costs will hasten the company's demise.

There is no net value in the company and the debt holders will probably kill it and delist.
