There is definitely connection between the exchanges.  Very often when I buy on OTC I can see the same transaction cross on the Canadian exchange - same volume, same exact time.  My sense is that when someone orders on OTC and shares are not available, the broker must first get them in Canada, then fill the OTC order in the US.  This is easy to spot on Stockwatch for example, by looking at the last ten trades for both exchanges and picking out the ones that match.  My brokers do not allow me to buy on exchanges in Canada, but I've done pretty well in arbitrage mode - watching for imbalances between the prices in Canada and the US and exploiting the differences.
