They- the brokerage house in my case- sent the proxy voting form/instructions.  A request form for interim financial statements and annual financial forms and a brief note that referred to an information circular on the website. 

The 25 page document  was not sent out and at the very least it should have been front and centre in the notice.  This is a far cry from the transparency that Roger kept promising.
