Bad

BadVery sad and tragic when anyone is killed trying to do a job.  Despite all the safety
regulations etc that are supposed to be in place,  some safety rule wasn't followed
or enforced.   Who's fault this was is too early to say but the bottom line was a death
of a worker.   This and leaks or spills are the absolute worst things that companies
in the sector have to do handle from a PR perspective.

I know when I worked in industry,  there were very strict protocols for every job and
safety staff gave daily orientation to employees before starting and then came around
to observe and inspect in order to make sure they were being observed and ahered
to.  If there were problems,  there was one warning and then  disciplinary action was
taken.   Everyone understood what was at stake.  Accidents do happen but they
should never result in a death.  Very tragic and disheartening to hear about this.
