for the last company he was employed with for over 22 years Peter Kiewit and Sons, did what axe did in terms of foreseeability that I am not at all pleased with happened, his a$$ would be so fired. You can't make these mistakes they are simply stupid!!! His company won a government bid to build a local bridge and they recommended to the government something that would add extra costs in regards to the cables it was suspended by which was to account for winter conditions and potentially avoid any ice bombs. The government chose not to heed the advice and sure enough there were ice bombs and blame went first to the company even though it built as to specifications and warned about this issue. This is what I am talking about. When you undertake things and parts are exposed to extreme heat and pressure electricity turned on and off they will also have a tendency to Crack shatter break due to expanding and contracting. This again is not rocket science and should have been foreseen there is no reason when doing proper risk assessment/management that there should be this oversight. But yeah anyone working as an engineer makes this type of mistake they would be fired!!!!
