Final numbers re: merger with UEC

Final numbers re: merger with UEC
As we approach tax time in Canada, I'm trying to put together the numbers I will need to calculate my new Adjusted Cost Base. However, with all the back and forth that occured last August, I don't have a very clear picture on the final numbers that went into the deal. What I do know for sure is that one UEX share was worth .09 of one UEC share. So the conversion ratio is simple enough. What I am missing is the following:
1. What were the dates and closing share price figures used in the calculations for both UEX and UEC?
2. What was the date and amount of US exchange spot price used in the calculation?
3. How do I calculate my new ACB?

Any assistance would be greatly appreciated.
