Where there is a jog in a fault that jog becomes a trap for gold once mineralized fluids flow through the fault. The geological feature is called a dilation. It may or may not include a large open cavity which subsequently gets filled with gold-rich Quartz. A common feature is called "ladder veins" which go across the jog. They are very prominant at the Fosterville gold mine. The central dilation of the KMZ produced the spectacular intercepts of 2021. I suspect that they are now into another dilation/jewelry box at Iceberg.
You don't come across a 2845 gm/t hole every day! The fact that it starts only 35 m down is a huge bonus. Something like this is easily accessed by an underground ramp.
