For scale, i looked up natural oil leaks in Eastern Canada oceans but couldn't find anything. For Gulf of Mexico they say 4000 to 17000 tonnes per year, roughly 3500 - 15000 m3/year:

Not trying to downplay it but even with all the multiple layers of procedures, safety model is still swiss cheese where very very occasionally holes line up and bang, 30 min leak.
