Dual eligible Medicare/Medicaid (25%) and Medigap (40%)  Medicare patients pay no deductible.  Many Medicare advantage plans (50%) also pay no deductible.  Greater than 65% of patients will pay no deductible and the number is probably 80% if you include Medicare advantage.
