a2bman, I don't have anything to fill out. Nothing in the mail, nothing online. Broker still trying to get the info.

I contacted ALL parties. Frontera, computershare, my broker. Computershare not holding my shares.

Strange how everyone else got stuff in the mail and online, yet my broker still waiting.
