In the past airline managers would have leeway to bend rules, but effectively, the fare you buy gives you what options you have for canceling and refunding tickets. That being said, have you looked at your credit card's certificate of insurance? Your credit card often has insurance that can cover for trip cancellation and medical reasons (eg. surgery) would often be covered.
