Interesting to see there is very little plastic deformation at the section where final breakage took place. This makes one think of a fatigue failure. As the VP said: "Like most kind of extreme events, it requires a chain of things to all happen, not just one thing." So: 1 A crack starting defect in a weld (acceptable, as per spec.) 2 A slight unballance in the rotor, some ...more

I am assuming that; 1 the cost of replacing the unit is covered by insurance 2 the lost revenue due to the collapse is also covered by insurance   or it is covered by the manufacturer Assuming assuming assuming .....
