NYUPN Clinically Integrated Network, LLC (NYUPN), is collaborating with
UnitedHealthcare to provide enhanced, coordinated care to more than
70,000 New York residents, and help to reduce costs through the region’s
largest shared savings initiative.
Under the new initiative, New York residents enrolled in
UnitedHealthcare’s employer-sponsored health plans will have access to
NYUPN’s coordinated care through its more than 1,700 clinical faculty
and other health care professionals affiliated with NYU Langone Medical
Center and the University Physicians Network (UPN), with a focus on
helping patients improve their health.

The 70,000 UnitedHealthcare employer-sponsored plan participants who
currently receive care from NYUPN care providers will not have to do
anything differently to receive the benefits of the collaboration, which
include enhanced care coordination and follow-up. Participating primary
care doctors will receive monthly updates on their patients, enabling
them to monitor all of the care the patient is receiving. This model
will help manage services for patients with chronic conditions, such as
diabetes and heart disease, while keeping a focus on health and
well-being by rewarding prevention, increased patient safety and
appropriate care utilization.
