3M Introduces Advanced Physician Documentation Platform

To help healthcare systems improve the accuracy of patient data and address growing compliance concerns with electronic health record (EHR) documentation, 3M today introduced new physician workflow technology that transforms the documentation process for physicians. With computer-assisted physician documentation guidance, automated clinical documentation improvement (CDI) queries, and a sophisticated “copy-paste” analysis tool, the new 3M 360 Encompass MD System makes it easy to capture accurate patient information, meet ICD-10 documentation requirements, and minimize compliance risk—all in real-time as the physician documents within the EHR.

Recent studies suggest that 30 to 70 percent of physician notes1 are duplicated from prior documents, which can lead to potentially dangerous copied forward or duplicate data in the patient record. “Documentation-assist features in the EHR provide improved efficiencies for physicians, but there are significant risks,” said Thomas Kravis, MD, physician executive, 3M Health Information Systems. “Instead of restricting the use of copy-paste, organizations can instead give physicians the tools they need to help them document accurately, respond quickly to CDI queries, and verify the accuracy of their documentation as part of their standard EHR workflow.”

3M Health Information Systems will showcase the new 3M 360 Encompass MD System at the HIMSS 15 Conference and Exhibition, April 12-16 at McCormick Place in Chicago. HIMSS attendees can view a full software demo at 3M booth #1212 in the South Exhibit Hall and access the new 3M white paper, “Addressing the Risk of Copy-Paste Documentation Errors.”

3M 360 Encompass MD integrates with all major EHR systems to analyze available patient data—including EHR templates and dictated documents—and then identifies unclear, unspecified or missing diagnoses and gaps in documentation. It helps physicians prepare for ICD-10 by prompting them for more specificity as they are documenting and only when more information is required to meet coding guidelines. The intelligent copy-paste analysis tool, scheduled for release in late summer 2015, differentiates between appropriately copied content from prior notes and content that is high-risk for compliance and should be reviewed by the physician.

3M 360 Encompass MD is an add-on module to the 3M 360 Encompass System, extending its natural language processing (NLP) benefits to physicians.
