For what I know, when HbA1c is over 6.5%, the first line of treatment is metfornin.

When a patient on metformin still over 6.5% of HbA1c, a second line of treatment is giving.

Those medications in second lines are insulin secretagogues, which cause the pancreas to produce and secrete insulin-and can lead to weight gain as a side effect.

Maybe the term "significant" is used simply because there a loss, not a gain in the waist circumference, and a reduction in the  body mass index.

I guess I will need an expert opinion for this one.
