Would you please endeavour to complete, sign and date these forms today, and submit them to your "CDS Participant", the broker, bank, trust company or securities company where you maintain your accounts in which the debentures are on record.

It would be most helpful if this can be done by the end of the day today, and a confirming e-mail sent to us so we can track our progress. If you have not received the e-mail and attached documents, please contact us ASAP. We have communicated with CDS Depository services to altert them to expect the paperwork to start arriving today.
