There is no work being conducted there until they feel that the site is safe. It was all state of the art equipment, but I cannot tell if they have made any determination as to what caused the accident.

You don't get the conclusion while the investigation is ongoing. An accident involving a winder in a mining shaft -- like a plane crash -- may be the result of a series of errors or miscalculations in design, workmanship, installation, use, maintanence, training etc. And who will take responsibility?
