Sirius Radio (SIRI) did, with even worse liquidity problems than Yellow has.

 

But SIRI had a dynamite CEO who proudly proclaimed that no one was going to take his company into Chapter 11, and he convinced one bond maturity after another to take equity.   It was a beautiful thing to behold.
