Generally, DOC has been trading ~1 to ~2 million shares a day on the CSE (
https://thecse.com/en/listings/life-sciences/cloudmd-software-services-inc), so that level of volume is fairly immaterial to how the Company will actually trade when the halt is lifted.
Total shares is total shares - shares traded OTC do not increase the amount of shares outstanding. To be honest, I don't know where this volume derives from, but I think it might have to do with different geographies (i.e., shares traded in the US). The price I am looking at on Google Finance is quoted in USD, which is way I came to this hypothesis (but by no means definitive!).
