If you scroll down its web page https://www.bonsecours.com/health-care-services/senior-services/assisted-living# you can find Search box "Assisted Living & Skilled Nursing Facilities Near Your our locations that can treat you. Certainly you can find nothing if it exits all snfs. There must be plenty of SNFs you can choose.

Here are difinitions of acute and primary care: "Acute Care Nursing treats rapidly-onset conditions often caused by a virus, infection, or injury resulting from trauma. Primary Care Nursing treats chronic conditions including diabetes, heart disease, high blood pressure, or asthma" So it seems primary care are SNFs.
