What caused the debt to spike?

I'm curious - what caused the debt to rise more than expected in the first place? If they didn't miss that target, none of the tanking would have happened. It seems to me that revealing more debt than expected triggered this whole mess....Was it just a late close on the sale of their $14M asset and slight overuns on the pennwest acquisition? Love to get some analysis on exactly what caused  the debt to spike.

Please don't reply with annoying 'because management sucks!' type posts. I'd really like to analyze this issue. Is there justification for accessing the amount of credit that they did?
