Exchange-reported volume is inaccurate?

Just for curiosity and educational purposes, usually you would never be able to tell on most tickers. I added 10K shares a bit before 3PM today (Scotia iTrade) and it didn't get included in the total volume. No reported volume between 2PM and 3PM on BNN Bloomberg or Yahoo Financials

Makes me wonder how many more trades occur behind-the-scene or unreported
