I am just on lunch break, so don't have time to read all of the most recent incidents reported, ...there are just too many of them. But...
Note that in at least one of the cases of contamination of a Q180V Olympus duodenoscope the Medivators reprocessor was used. This happens to be one of the only reprocessors that the FDA feels meets its standards for high level disinfection of this most commonly used scope at this time. 
Also take note of the number of non-duodenoscope problems, involving bronchoscopes and colonoscopes for example, that are now being reported. 
Problems seem to exist for all types of reprocessors, especially including Olympus's own product, and some are, again, related to improper or inadequate reprocessor technician techniques and inspections of devices.
Unfortunately for our patients, health care providers, and the FDA, these ongoing problems are just the tip of the iceberg.
And, the VP4 is a cost-effective, efficient, and terminal sterilization solution for most if not all of these issues, (in Canada, but somehow the efficacy south of the border remains to be recognized?). Hope the FDA gets moving a little, or a lot, faster!!
